Target Clientele: |
Impaired adults or seniors who have a physical or mental impairment that limits the activities
of daily living and have a health condition requiring treatment and have a health condition requiring treatment
or rehabilitative service prescribed by a physician. |

Description of Activities: |
The center’s activities are designed to meet the need of the total person. Emphasis
is placed on social interaction, educational topic, health and safety issues, mental and physical stimulation. |

Rate Structure: |
Services are covered by Medi-cal. A sliding fee scale is available for those individuals not
eligible for Medic-cal. Scholarships are available to uninsured seniors who reside in the City of San Jose. |
